Request Form Instructions

1. Download Form

2. Fill out Form To Include All Pertinent Data

  • Check boxes for require tests.
  • If needed include PO number.
  • Include the desired due date for results at the top right-hand corner.
  • Cement Lab Only:  Slurry Design Info – Design info must include concentrations for a lab weight sheet. If not providing grams, ensure you have provided SG of cementitious additives (such as Poz) and whether we need to calculate lab weights based on percent BWOB or BWOC calculations.

3. Submit Form

Submit the form to the appropriate emails listed for each lab.

4. Submission Confirmation

A team member will contact you with confirmation that your form has been received.

5. Delivering Chemical Samples

All chemicals sent must be marked with chemical name/ID, lot numbers, and SDS.

A drop box is located to the right of our front door for after hour delivery.
